Our Referral Process

It is important we have as much information as possible about the client you are referring. Below you will find a sample list of clinical/medical information that will be extremely helpful for us. You can find the full version by visiting our online form. This form can be submitted on line or faxed to 702-515-1379

  • Patient name, age, sex
  • Is patient medically stable?
  • Pain Diagnosis, Surgical History
  • Axis I, Axis II, Axis III, Axis IV, Axis V
  • Medication dosage, reason for prescription, dates prescribed
  • History of SI, previous Suicide attempts or self mutilation
  • Immediate family contact information
  • Insurance plan and private pay ability

Because Las Vegas Recovery Center treats medically compromised and complex cases, additional medical records may be requested. This can include summaries from any other programs your client has participated in, surgeons, pain management specialists, MRI’s, and X-Rays.

As many in our field are aware, many insurance companies provide less then favorable authorizations. The admissions office will be very upfront regarding this portion to prevent any surprises. Pre-authorizations for insurance can only be obtained once your referral as been assessed at our center. Authorizations for our Pain Recovery program are extremely rare and occur in less than 1% of our admissions. Please see the list of insurance companies that are “in-network” for us included workers compensation contracts.

If your referral schedules an admission, please inform the admissions staff of the type of reporting you would prefer. This includes frequency of reports and the best way for you to receive them. An initial call from the intake department will occur to inform you the client has admitted. The admissions office will coordinate and ensure all clinical and medical information is delivered to the treatment team prior to your client’s admission. If you are an interventionist that is traveling with your referral to admission, we will ensure you are returned to the airport or a local hotel property of your choosing.

Provided your referral signs appropriate releases, you will be given names and contact information for the client’s primary and secondary counselor within 24 hours of admission. Consistent communication will continue into discharge planning and actual discharge. If for someone reason you are not receiving updates from us, more than likely the client has revoked a previously signed release. In this instance, the matter becomes a clinical issue that will be addressed.
